Job summary

Trustbridge Hospice, a member of Empath Health, seeks a caring RN Hospice Case Manager to join our team in Central Palm Beach County. Schedule: PRN 8:30 a.m.–5 p.m. with weekend availability; you will support patient-centered care and work within the interdisciplinary group.

Florida-licensed RN with BSN preferred and hospice certification preferred, plus current BLS. Strong assessment, teaching, and communication skills are essential; EMR experience is a plus and reliable transportation is

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
• Understanding of and ability to apply Hospice Nursing: case management, crisis intervention, communication, teaching, thanatology, and needs.
• Basic computer skills; EMR experience a plus.
• Reliable transportation with valid Florida driver’s license.

Responsibilities

• Coordinate patient/family care under physician and RN supervision.
• Identify patient/family needs and contribute to an individualized plan of care.
• Collaborate as part of the interdisciplinary group (IDG).
• Utilize monitoring, intervention, and teaching to maximize comfort and quality of life.
• Maintain confidentiality in care of patient/family.

Trustbridge Hospice, a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Central Palm Beach County. You would work the following schedule: PRN 8:30 a.m.- 5 p.m., with weekend availability.

What You'll Do

• Support and carry out the organization’s mission, vision, and values on a consistent basis.
• Coordinates the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG), contributes to and follows an individualized plan of care
• Utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere, and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintain utmost respect for confidentiality in the care of the patient/family.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills, and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
