Sanctuary Hospice seeks a Per Diem Hospice RN Case Manager to support patients and families in Wheaton, Warrenville, Lisle, and surrounding communities. The RNCM delivers hands-on hospice nursing, coordinates services, and guides families through the journey with compassion and clinical excellence.

Competenze

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
