Job summary

Legacy Hospice in Charlottesville is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through every step of their hospice journey with a focus on dignity and comfort.

As part of our interdisciplinary team, you’ll educate families, respond to referrals, and participate in call duties while upholding clinical excellence and compassionate care.

Qualifications

Hold current unencumbered license as a Registered Nurse.
Minimum one year experience in medical-surgical/acute care; hospice exp a plus.
CPR certification if required by state; valid driver’s license and transportation.
Proficient with EMR and basic computer tasks.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Deliver direct care coordination to maintain patient comfort and quality of life.
Educate patients and families to create personalized care plans.
Share call duties with the care team.
