Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager to join our dedicated nursing team delivering compassionate end-of-life care throughout the Naperville community. The schedule is Monday through Friday, 8 am to 4:30 pm, with a focus on coordinating services and guiding patients and families through their hospice journey.

Formación

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
