Job summary

Sanctuary Hospice is seeking a compassionate RN Case Manager to join our per diem nursing team. You will coordinate hospice services, deliver hands-on care, and guide patients and families through the care journey in Summit, Portage, Medina, Stark and Wayne counties.

The role requires an active RN license, 1 year of medical-surgical experience, and proficiency with EMR. Per diem schedule offers flexibility with on-call duties and a strong team support system.

Qualifications

• RN license active and in good standing.
• Minimum 1 year experience as an RN in medical-surgical/acute care; hospice experience a plus.
• CPR certification; valid driver's license and dependable transportation.
• Proficient with computer applications and electronic medical records.

Responsibilities

• Respond to clinical referrals with care and expertise, addressing inquiries with compassion.
• Partner with Clinical Director to assess client eligibility and suitability for hospice services.
• Deliver hands-on hospice nursing care and coordinate services for patients at home or community settings.
• Provide nursing care rooted in established principles to ease pain and improve quality of life.
• Educate patients and families to participate in personalized care plans aligned with goals.
• Participate in on-call duties to ensure timely support when it matters most.
