Job summary

As an RN Case Manager, you will collaborate with our interdisciplinary team, participate in education, and contribute to a supportive onboarding environment with benefits and growth opportunities.

Qualifications

• Current unencumbered RN license.
• At least 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient computer skills with EMR experience.
• Current CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Coordinate direct patient care while ensuring comfort and quality of life.
• Deliver nursing care rooted in proven principles.
• Educate patients and families to create personalized care plans.
• Share call duties to be available when it matters most.
