Job summary

Heart of Hospice, a part of LHC Group and UnitedHealth Group family, is seeking a Per Diem RN Case Manager to deliver the Plan of Care for hospice patients in Louisiana. The role involves nursing evaluations, care planning, and timely documentation to ensure high-quality patient care.

The ideal candidate holds an active RN license, has at least 1 year of clinical experience, and a valid driver's license. CPR certification is preferred.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
