Qualifications

Current and unrestricted RN licensure in the state of practice.
Current driver’s license and reliable transportation.
CPR certification or ability to obtain within 90 days.

Responsibilities

Make initial nursing evaluation to determine hospice eligibility within 48 hours.
Identify and reassess patient/family needs at least every 15 days.
Initiate and coordinate the plan of care with the IDG.
Document patient progress and responses to hospice care.
Collaborate with patient/family, physician and care team daily.
Instruct patient/family in self-care and maintain clinical notes.
Provide care in home settings regardless of client demographics.
