Job summary

UnitedHealth Group seeks a registered nurse to join Heart of Hospice, a part of LHC Group, delivering compassionate, home-based hospice care in Louisiana. You will perform initial assessments within 48 hours of referral and coordinate the plan of care with the IDG, while supporting patients and families through care planning and education.

The role requires an unrestricted RN license in Louisiana, valid driver's license, and reliable transportation.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• 2+ years of full-time RN experience for LA state.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents problems, goals, interventions, and responses.
• Collaborates with the patient/family, attending physician and IDG to deliver daily patient/family care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on the patient’s condition.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
