Job summary

Heart of Hospice, a part of LHC Group and within the Optum family, seeks a Per Diem RN Case Manager to deliver the Plan of Care for hospice patients in Louisiana. You will regularly evaluate performance to improve productivity and care quality.

Responsibilities include initial eligibility evaluation, identifying needs, coordinating care, documenting outcomes, and collaborating with the care team to support patients and families.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions and patient/family response to care.
• Collaborates with patient/family, attending physician and IDG to deliver daily care.
