Job summary

Sanctuary Hospice is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) for per diem coverage. The role covers Wheaton, Warrenville, and Lisle areas with a Monday–Friday schedule, 8 am to 4:30 pm, and involves hands-on hospice nursing, care coordination, and family education.

You’ll work with the interdisciplinary team to ensure patients receive high-quality, compassionate end-of-life care, while following best practices and maintaining documentation in EMR.

Qualifications

Current unencumbered RN license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
Proficient in electronic medical records (EMR).
Valid driver's license and access to reliable transportation.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Deliver hands-on hospice nursing care and coordinate services.
Educate and advocate for patients and families in care planning.
Participate in call duties and after-hours coverage.
