Job summary

Holisticare Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing compassionate end-of-life care throughout the community. This on-call role supports on-call triage and on-site patient visits in Lehigh & Northampton counties, with a per diem schedule.

You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey, working with an interdisciplinary team to ensure

Qualifications

• Hold current unencumbered RN license.
• Minimum one year experience as an RN in a medical surgical/acute care setting; Hospice exp a plus.
• Must be computer proficient in typing and EMR.
• Possess and maintains CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with Clinical Director to assess eligibility and suitability of clients for hospice services.
• As an RN Case Manager, coordinate care and provide comfort and support for patients to maintain quality of life.
• Deliver top-notch care rooted in proven nursing principles and compassion.
• Educate patients and families to create personalized care plans aligned with goals.
• Share in call duties to be available when it matters most.
