Job summary
Sanctuary Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) to join our caring nursing team. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through every step of their hospice journey in Summit, Portage, Medina, Stark and Wayne Counties.

Qualifications
- Hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient with computers and EMR systems.
- CPR certification if required by state.
- Valid driver's license, vehicle insurance, and reliable transportation.
Responsibilities
- Respond to clinical referrals with expert, compassionate care.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Provide direct nursing care to maintain patient comfort and quality of life at home.
- Deliver nursing with heart, grounded in proven principles and compassion.
- Educate patients and families to develop personalized care plans.
- Share call responsibilities as part of the team.
