Responsibilities

• Provides and manages direct patient/family care as part of IDT.
• Coordinates Plan of Care reflecting evolving needs.
• Educates patients and families about disease, palliative options, and safety.
• Visits patients at home; documents changes and coordinates referrals.
• Maintains professional conduct and collaborates with IDT during rounds.
