Job summary

Chapters Health System is seeking an RN Case Manager in Virginia to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative, and supportive care to keep the patient in their home environment as long as possible.

The role requires an active RN license, 1 year of nursing experience (hospice or hospital preferred), EMR/EHR experience, a valid driver’s license, and strong communication skills in English.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience, hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance.
• Able to visit participants in their homes and meet visit schedules.
• Active BLS for healthcare professionals.

Responsibilities

• Provide direct care to patients and families as part of the IDT.
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patients and families about disease, palliative care, safety and caregiving.
• Perform home visits to assess safety, medication and nutritional needs.
• Communicate changes in patient condition to IDT and physicians.
• Participate in IDT meetings and present concise reports.
• Supervise care provided by home health aides and CHWs.
