Job summary

Hospice of Virginia in Richmond, VA is seeking an RN Case Manager to support patients and families facing end of life. You will assess needs, plan and coordinate home-based nursing care, and collaborate with the interdisciplinary team to ensure compassionate, quality care.

Shift Information: Monday–Friday 8a–5p with rotating on call. Comprehensive benefits offered including medical, dental, vision, 401(k) with match, PTO, tuition reimbursement and holidays.

Qualifications

• Graduate from an accredited nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Knowledgeable in quality assessment and assurance procedures.

Responsibilities

• Assume primary responsibility for patient and family caseload across the nursing process: assessment, eligibility, admission, planning, implementation and evaluation.
• Gather data from patient and family to determine needs and coordinate with interdisciplinary team.
• Communicate with attending physician and other agencies to coordinate optimal care.
• Provide regular home visits to reassess needs and update care plans.
