Job summary

VHS Hospice in Newport News, VA is seeking an RN Case Manager for a full-time, all-inclusive role. You will coordinate care across the continuum, assess patients, develop and monitor care plans, and advocate for residents in collaboration with interdisciplinary teams.

Qualifications

• RN with ADN required; BSN preferred.
• Strong communication, negotiation, and problem-solving abilities.
• Solid understanding of healthcare regulations and care coordination principles.

Responsibilities

• Coordinate and manage care for patients with chronic, acute, or complex conditions.
• Conduct comprehensive patient assessments to identify needs.
• Develop, implement, and evaluate individualized care plans with teams.
• Monitor progress and adjust plans as needed.
• Advocate for residents and ensure preferences are respected.
• Facilitate communication among patients, families, physicians, and providers.
• Schedule and coordinate appointments, referrals, and transportation.
• Ensure efficient use of resources while maintaining quality care.
• Educate patients and families about diagnoses and options.
