Hospice of Virginia is hiring an RN Case Manager to support Richmond and surrounding counties. Join our compassionate team of clinicians delivering end-of-life care with dignity and personalized planning.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed RN in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage patient and family caseload across assessment, eligibility, admission, planning, implementation and evaluation stages of the nursing process.
• Collect data from patient and family to identify needs and coordinate with the interdisciplinary team.
• Coordinate communication with attending physician and other agencies to ensure optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans; document delivery of care by Home Health Aide as required.
