Job summary

Bristol Hospice in Richmond, VA is seeking an experienced RN Case Manager to provide compassionate, holistic end-of-life care. You will develop individualized care plans, coordinate with families and interdisciplinary teams, and support patients through every step of their journey.

This full-time position offers a day shift with a salary range of $90,000 to $95,000 per year, mileage reimbursement, and comprehensive benefits.

Qualifications

• RN license in good standing in the state of practice.
• CPR certification current and valid.
• Strong communication and observational skills.
• Flexible, team-oriented attitude.

Responsibilities

• Develop and implement individualized care plans based on medical history and current status.
• Deliver one-on-one support to patients and families with dignity and compassion.
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document accurately (temperature, pulse, respiration, BP).
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Track changes in condition and respond promptly with interdisciplinary teams.
• Maintain up-to-date clinical notes reflecting patient progress and quality of care.
