Job summary

Ascend Hospice & Palliative Care VA is hiring an RN Case Manager to support on-call coverage, assessments, and care coordination in the Richmond/Glen Allen area. This role involves patient visits, collaborating with the care team, and ensuring compassionate end-of-life care.

Hours are Monday through Friday, 8:30am–5:00pm, with travel as needed. The ideal candidate holds an RN license, has hospice experience, and strong communication skills. We offer competitive salaries and benefits.

Qualifications

• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to coordinate care.
• Counsels the patient and family in meeting end-of-life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient's needs and reports changes in condition.
• Supervises the Hospice Aides.
