Job summary

Ascend Hospice & Palliative Care VA in Richmond/Glen Allen, VA is hiring an RN Case Manager. The role involves on-call coverage, case management, and patient visits Monday–Friday.

Must hold a State Certified RN license and have at least 2 years of professional nursing experience, with hospice experience preferred. The team emphasizes compassionate, continuous, well-coordinated care, with opportunities for growth and a comprehensive benefits package including healthcare, 401(k), PTO, mileage

• Complete initial patient/family assessment to determine hospice care needs.

• Initiate preventative and palliative nursing procedures.
• Communicate with physician about patient needs and changes.
• Supervise Hospice Aides.

Now Hiring – RN Case Manager (Richmond/Glen Allen, VA)

Hours: Monday - Friday 8:30am - 5:00pm

TheRN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides
