Job summary

Advanced Home Health and Hospice in Reno, NV seeks a dedicated Hospice RN Case Manager to coordinate patient care in a hospice setting. You will provide skilled nursing care, manage care plans, and collaborate with an interdisciplinary team to honor patient wishes.

The role requires an RN license, 2+ years of experience, CPR certification, and reliable transportation for home visits. Hospice experience is preferred; EMR systems experience is a plus.

Qualifications

• RN license in good standing in state of practice.
• Minimum of 2 years as an RN.
• Hospice or related healthcare setting experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of hospice regulatory requirements and documentation.
• Experience with EMR systems is preferred.

Responsibilities

• Conduct comprehensive patient assessments to identify healthcare needs and develop care plans.
• Provide skilled nursing care including pain and symptom management per care plans.
• Coordinate and supervise hospice aides and other healthcare professionals.
• Communicate progress and treatment plans with patients, families, and the care team.
• Monitor patient wellbeing and ensure comfort and dignity in end-of-life care.
• Collaborate with the interdisciplinary team to align care with patient wishes.
• Ensure regulatory compliance and proper documentation.
• Provide bereavement support to families as needed.
• Participate in team meetings to discuss progress and barriers.
