Job summary

New Century Hospice seeks an experienced Hospice RN to lead patient care in home settings. You will assess patients, implement skilled nursing interventions, and collaborate with a diverse care team to refine individualized plans of care.

You will educate families, monitor symptoms, document accurately, and support transitions between care levels. A state RN license and CPR are required, with hospice experience preferred.

Qualifications

• RN licensed in state of employment.
• CPR certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic evaluations.
• Deliver and document skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Educate patients and caregivers on disease processes, end-of-life care, medications, nutrition and self-care.
• Communicate condition changes to the care team promptly.
• Support continuity of care across home health, palliative and hospice transitions.
• Document timely clinical records including orders, progress notes and care plans.
• Participate in on-call and patient support activities as applicable.
