Advanced Home Health and Hospice is seeking a dedicated Hospice RN Case Manager in Carson City, NV. The role coordinates and manages patient care in a hospice setting, providing skilled nursing services and implementing care plans with an interdisciplinary team.

The position requires an active RN license in the state, CPR certification, and strong clinical assessment, communication, and time-management skills.

Qualifications

• RN license required and CPR certification preferred.
• Minimum of 2 years of RN experience.
• Hospice or related healthcare setting experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including pain and symptom management per orders.
• Coordinate and supervise hospice aides and other professionals.
• Communicate with patients, families, and physicians about progress and plans.
• Monitor patient well-being and ensure comfort and dignity in end-of-life care.
• Collaborate with the interdisciplinary team to align care with patient goals.
• Ensure regulatory compliance and accurate documentation for hospice care.
• Provide bereavement support to families and caregivers.
• Participate in team meetings to discuss care plans and barriers to goals.
• Perform duties assigned by Hospice Director or supervisor.
