Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Current CA RN license and CPR certification.
• Minimum of 2 years licensed experience in California.
• Bachelor’s degree with one year of home health care experience preferred.

Responsibilities

• Completes initial and ongoing patient assessments and histories.
• Develops and revises care plans based on patient needs.
• Administers medications and treatments as prescribed by physician.
• Counsels patients and families on nursing and related needs.
• Coordinates patient care as Case Manager when assigned by Clinical Supervisor.
• Maintains accurate clinical notes and updates physician as required.
