Job summary

Mission Healthcare, a leading home health and hospice provider in the Western United States, is seeking a Full-Time Registered Nurse in the Greater Riverside/San Bernardino area. You will perform initial patient assessments, develop and adjust plans of care, and deliver skilled nursing services in patients' homes.

This role requires an active California RN license, BLS, and at least one year of acute care experience; a BSN is preferred.

Qualifications

• Current and valid state RN license.
• Bachelor’s degree in Nursing.
• 1 year of acute care experience preferred.

Responsibilities

• Initial patient and family assessment to determine home care needs.
• Initiate and revise care plan as patient status changes.
• Regularly re-evaluate patient needs.
• Use health assessment data to determine nursing diagnoses.
• Develop goals and care plan with patient and family.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family on plan of care.
• Provide health instructions per assessment and plan.
• Identify discharge planning needs before discharge.
