Qualifications

• AA or B.S. degree in Nursing from an accredited nursing program.
• Current license as a registered nurse in California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Responsibilities

• Provide in-home nursing care as part of an interdisciplinary team including nurses, social workers, chaplains, hospice aides and physicians.
• Coordinate patient/family services and prioritize needs, implementing the individualized plan of care.

Education

California RN license

Public Health Nursing certification desirable

HPNA certification preferred
