Job summary

Anchor Health is seeking an experienced Hospice RN Case Manager to join our team in California. The role involves conducting comprehensive patient assessments, performing home visits, initiating and revising care plans, and coordinating with families to ensure high-quality end-of-life care.

The position requires an accredited nursing degree, an active California RN license, and CPR certification, with prior hospice/home care experience preferred.

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR certificate.
• Minimum of one year of nursing experience within the last three years or a BSN with RN license.
• Must have a valid California driver license and insured vehicle.
• Strong observation, verbal and written communication skills.

Responsibilities

• Completes initial and ongoing patient assessments to determine hospice needs.
• Provides comprehensive physical assessment and patient history.
• Performs home visits and evaluates patient status.
• Initiates and revises the plan of care based on patient needs.
• Develops a care plan involving patient and family with nursing actions.
• Administers prescribed medications and treatments.
• Participates in Interdisciplinary Group Meetings.
• Documents patient condition and care provided.
• Supervises ancillary personnel as needed.
