Job summary

Mission Healthcare in California is seeking a part-time Registered Nurse for home health visits, Saturdays and Sundays, 8:00am-6:30pm. You will perform initial patient assessments, develop and revise care plans, and provide caregiver education while coordinating with the broader care team.

The ideal candidate holds a current RN license, at least 1 year of acute care, BLS certification, and a valid driver's license with auto insurance.

Qualifications

Current and valid state RN license.
Minimum of 1 year of recent acute care experience in an institutional setting.
Basic Life Support (BLS), with hands‑on CPR certification.
Valid driver's license and auto insurance.
Bachelor’s degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

Completes initial patient and family assessment to determine home care needs and documents findings.
Initiates and revises the plan of care as patient status changes.
Regularly re-evaluates patient needs and adjusts interventions.
Uses assessment data to determine nursing diagnoses and priorities.
Develops and updates care plans with goals based on diagnoses and patient/family input.
Administers medications and treatments as prescribed and counsels on plan-of-care.
