Job summary

HealthFlex Home Health & Hospice is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to provide skilled services to patients and families at end of life in a home health setting in Northern California. Schedule options include Full Time, Part Time or Per Diem.

Responsibilities include comprehensive assessments, planning and coordinating care with the interdisciplinary team, administering medications, and educating families.

Qualifications

• Current and unrestricted CA license.
• 1+ years Hospice Case Manager experience.
• AHA CPR certification.
• Driver's License.
• Travel to Oakland office for weekly IDG meetings.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Write and initiate plan of care; regularly re-evaluates patient and family/caregiver needs.
• Participates in revising the plan of care as necessary.
• Use health assessment data to determine nursing diagnosis.
• Develops a care plan based on nursing diagnosis with patient and family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Provides health care instructions to the patient and family.
• Assist patient with activities of daily living and support self-sufficiency and comfort care.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Completes and submits accurate clinical notes and pain/symptom management changes.
• Communicates with physician regarding changes in patient condition and orders.
• Teaches patient and family self-care techniques and health counseling.
• Works with Interdisciplinary Group to meet emotional needs.
