Job summary

Heartland Home Health & Hospice LLC seeks an experienced Registered Nurse to plan, organize, and direct home-based hospice care with a palliative focus. The role emphasizes community health education and collaboration with families to meet needs in home settings.

The RN will perform comprehensive assessments, develop and revise care plans, administer medications, teach patients and families, and coordinate with the interdisciplinary team to ensure continuity of care.

Qualifications

• Graduate of an accredited school of nursing; 1–2 years acute care in an institutional setting and 1–2 years in home care.
• Current licensure in State and CPR certification.
• Must be a licensed driver with insured vehicle in good working order.
• Bachelor’s degree with 1 year hospice care experience preferred.
• Management experience not required; may supervise hospice aides.
• Strong observation, verbal and written communication, problem solving and math skills; nursing skills per competency checklist.
• Prolonged walking/standing; ability to lift and transfer patients and lift supplies; visual/hearing acuity for nursing skills.

Responsibilities

• Completes initial, comprehensive, and ongoing assessments of patient and family; develops plan of care.
• Provides professional nursing care using the nursing process as defined by state act.
• Regularly re-evaluates patient and family needs; revises the plan of care as necessary.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnosis and establishes goals with patient/family.
• Administers medications/treatments per physician’s plan and counsels patient/family.
• Teaches self-care techniques; ensures safe environment and continuity of care.
• Acts as Case Manager; coordinates care for assigned caseload.
• Completes clinical notes and communicates with hospice/IDT; coordinates with community health professionals.
