Job summary

Qualifications

• Graduate of an accredited school of nursing.
• One year nursing experience recommended, hospice care experience preferred.
• Current licensure in state, CPR certification and valid driver’s license.
• Ability to supervise hospice aides.
• Strong observation, verbal and written communication skills.

Responsibilities

• Completes comprehensive assessments and plans of care for hospice patients at home.
• Assesses patient and caregiver needs and determines appropriate level of care.
• Develops and revises care plans with interdisciplinary team.
• Coordinates patient care and ensures continuity across services.
• Provides hospice education to patient, family, and staff.
