Job summary

Crossbridge Hospice in Troy, MI is seeking an experienced RN Case Manager to deliver end-of-life care in patients' homes and community settings. You will coordinate and supervise hospice services, educate staff, and partner with families to ensure compassionate, patient-centered care.

This role requires an unrestricted RN license in Michigan, a valid driver's license and auto insurance, plus at least two years of RN experience with hospice or palliative nursing preferred.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

• Provide end-of-life care and hospice services in home or community settings.
• Direct, supervise, and instruct hospice aide staff in personal patient care.
• Support patients and families through highly sensitive and meaningful experiences.
• Maintain and complete care coordination notes and visit documentation.
• Assist or arrange delivery of medications and supplies after hours, weekends, and holidays.
• Attend interdisciplinary team meetings during on-call periods for team discussion.
