Job summary

Crossbridge Hospice is seeking a dedicated Nurse Practitioner in Troy, MI to conduct face-to-face hospice assessments, collaborate with RN Case Managers and the interdisciplinary team, and ensure patient needs are met with compassionate, patient-centered care. The position requires a Master's in Nursing, an unrestricted NP license in Michigan, and board certification.

Hospice experience is preferred, with a valid driver's license and reliable transportation.

Qualifications

Master’s degree in nursing required.
Current, unrestricted NP license in the state of practice.
Board certification required.
Hospice experience preferred.
Valid driver’s license with good driving record.

Responsibilities

Conduct Medicare-required face-to-face encounters prior to the 180th day recertification and at each subsequent recertification.
Document clinical findings from face-to-face visits, including decline in clinical status and relevant comorbid conditions.
Communicate all clinical findings to the physician responsible for recertifying the patient.
Complete and submit attestation statements confirming the dates of each face-to-face encounter.
Submit time and mileage logs for all patient-related activities to the Medical Director every two weeks.
Ensure consistent use of the 4Ms—What Matters, Medications, Mentation, and Mobility—to provide Age Friendly Care.
