Job summary

Crossbridge Hospice is seeking an RN Case Manager to provide end-of-life care and coordinate services for patients in home or community settings. The role directs staff, supports families, and ensures compassionate, patient-centered care with a focus on quality and safety.

Based in Troy, MI, the position offers regular weekday hours with on-call as needed, driving a patient-first approach and collaborating with the clinical team to create individualized care plans that honor patient goals and

Qualifications

• Current unrestricted RN license in Michigan.
• Two years of RN experience; hospice experience preferred.
• Valid Driver's License and auto insurance.

Responsibilities

• Provide end-of-life care in patients' homes or community settings.
• Supervise and instruct hospice aides in personal patient care.
• Manage patient/family caseload and assess care needs during on-call hours.
• Coordinate care with the triage team and attending physicians.
• Maintain care coordination notes and visit documentation.
• Assist with medication delivery after hours or holidays.
