Job summary

Crossbridge Hospice in Troy, MI is seeking an On-call RN Case Manager to deliver end-of-life care in patients' homes or community settings. You will provide intermittent skilled nursing services and supervise hospice aides, guiding care with compassion and clinical judgment.

This role requires clear communication, strong clinical assessment, and collaboration with the triage team and physicians. You’ll coordinate medications, visits, and documentation while respecting patient goals and ensuring

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license and proof of auto insurance
• Two years of RN experience
• Two years of Hospice/Palliative Nursing experience preferred

Responsibilities

• Provide end-of-life care and hospice services in patients' home/community settings.
• Supervise and instruct hospice aides in personal patient care.
• Maintain and complete care coordination notes and documentation.
• Coordinate medications and supplies after hours, weekends, holidays.
• Attend interdisciplinary team meetings during on-call periods for patient/family needs.
• Admit patients eligible for hospice services under physician orders.
• Report patient status and needs to the clinical leadership and hospice physicians.
• Develop patient care plans addressing identified problems and goals.
