Job summary

Crossbridge Hospice is seeking an On-call RN Case Manager in Troy, MI to deliver end-of-life care in home or community settings. You will supervise hospice aides, assess patient needs, and coordinate nursing care under physician orders.

The role requires compassion, strong clinical judgment, and excellent communication to support patients and families during highly sensitive times. Schedule: 7 on/7 off; Mon-Fri 5pm-8am, weekends 24 hours.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

• Assume primary patient/family caseload during on-call hours with triage team.
• Report On-Call activities to clinical leadership and Branch RN Case Managers.
• Maintain and complete care coordination notes for visit notes and other clinical information.
• Assist, provide, or arrange delivery of medications and supplies after business hours, weekends, and/or holidays.
• Attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needs.
• Under the physician’s order, admit patients eligible for hospice services in collaboration with the triage team.
• Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician.
• Develops patient care plan addressing identified nursing problems and goals with patient(s) during on-call hours, weekends and/or holiday(s).
• Be responsible to ensure the use of the 4Ms and provide Age-Friendly Care.
