Job summary

Trinity Health at Home Hospice in Oakland County, MI, is seeking an RN Case Manager for part-time PRN assignments. Provide one-to-one nursing care in patients' homes and coordinate hospice services.

The role emphasizes compassionate care, care planning and symptom management, with collaboration from the physician and hospice team. Prior home health or hospice experience preferred, with a valid Michigan RN license and reliable transportation.

Qualifications

• Current Registered Nurse licensure in the state of practice.
• Must have a minimum of one (1) year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by the physician and in collaboration with the client/family and state requirements.
• Serve as the case manager for hospice patients and coordinate care plans.
• Collaborate with physicians and other hospice team members to anticipate and manage patient symptoms.
• Provide support to Hospice Aides and LPN/LVNs as needed.
