Job summary

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

• Provide end-of-life care to patients in home or community settings.
• Supervise hospice aides in the delivery of patient care.
• Coordinate care plans and communicate with the clinical leadership.
• Document care activities and patient status.
• Assist with medication and supply delivery after business hours or weekends as needed.
• Participate in interdisciplinary team meetings during on-call periods.
