Job summary

Crossbridge Hospice, LLC is seeking a compassionate RN Case Manager to deliver end-of-life care in patients’ homes or community settings. You will oversee intermittent skilled nursing services, supervise hospice aides, and guide families through meaningful, highly sensitive care experiences.

Candidates should have an active RN license, a valid driver's license, and at least two years of RN experience with Hospice/Palliative nursing preferred.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

• Assume primary responsibility for the patient/family caseload during on-call hours in collaboration with the triage team.
• Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift.
• Maintain and complete appropriate care coordination notes for visit notes and other clinical information.
• Assist or arrange delivery of medications and supplies after hours, weekends, and/or holidays.
• Attend interdisciplinary team meetings during on-call periods for team discussion and evaluation of patient needs.
• Admit patients eligible for hospice services under physician orders in collaboration with the triage team.
• Report patient status and needs to clinical leadership and hospice physicians.
• Develop patient care plans addressing identified problems and goals with patients and families.
• Ensure use of the 4Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care.
