Job summary

Gentiva Hospice in Augusta, ME is seeking a Registered Nurse (RN) to provide in-home hospice care on a Monday–Friday dayshift basis. You will assess patients, develop and document Plans of Care, and coordinate with the interdisciplinary team to ensure compassionate, individualized care for patients and families.

Candidates must hold an active RN license in the state, have at least 1 year of RN experience (hospice/home health preferred), CPR certification, and a valid driver’s license with auto

Qualifications

• Must have active RN license in the state of employment.
• Minimum of 1 year RN experience in acute care, hospice, long-term care, or home health.
• Hospice or palliative care experience preferred.
• CPR Certification required.
• Current automobile insurance and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education for patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Communicate changes in condition to the care team for timely interventions.
• Document timely clinical records and maintain regulatory compliance.
• Participate in on-call and patient support activities as applicable.
