Job summary

Crescent Hospice is seeking a full-time Hospice RN Case Manager to join our patient‑centered team in Augusta. You will deliver hands-on nursing care, coordinate services, and guide patients and families through the hospice journey with compassion and clinical excellence.

This role reports to the interdisciplinary care team, with a schedule Monday–Friday, 8 am–4:30 pm, and a market-aligned salary range. We offer a strong onboarding program and a comprehensive benefits package to support growth

Qualifications

Active RN license in good standing.
At least 1 year RN experience in medical‑surgical/acute care; hospice a plus.
Proficient with EMR systems.
Current CPR certification if required by state.
Valid driver's license and reliable transportation.

Responsibilities

Empower through engagement: respond to referrals with care and expertise.
Collaborative care: work with Clinical Director to assess client eligibility.
Direct care: deliver nursing care focused on patient comfort and quality of life.
Nursing with heart: apply proven nursing skills to ease pain and provide solace.
Educator and advocate: educate families and involve them in care plans.
Call of compassion: participate in call duties to support timely care.
