• Deliver and document skilled nursing care per Plan of Care in patient homes or facilities.
• Collaborate with interdisciplinary team to develop and update care plans.
• Educate patients and families on disease process, care needs, and end-of-life planning.
• Maintain accurate clinical documentation and ensure infection control/compliance.
