Job summary
Hospice of New Mexico is seeking a Full-Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team in Albuquerque. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey, with a Monday–Friday 8am–5pm schedule and on-call/weekend rotations.

Qualifications
- Hold current unencumbered RN license.
- Minimum two (2) years RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient in computer use, typing, and EMR systems.
- CPR certification if state requires it.
- Valid driver’s license, vehicle insurance, and reliable transportation.
Responsibilities
- Respond to clinical referral information with care and expertise.
- Collaborate with Clinical Director to assess eligibility for hospice services.
- Provide direct nursing care and coordinate services to maintain patient comfort and quality of life.
- Deliver nursing care grounded in proven principles and techniques to ease pain and bring solace.
- Educate patients and families to participate in personalized care plans.
- Share on-call duties as part of the team.
