Ambercare Hospice in Santa Fe, NM seeks a compassionate Registered Nurse Case Manager to deliver expert clinical care and coordinate patient and family support across the continuum. You will manage a caseload with autonomy in planning and visits, promoting comfort and quality of life for patients and families.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data and assess physical, psychological, social, and spiritual factors impacting needs and initiate interventions with the team.
• Coordinate care by communicating with the attending physician and interdisciplinary team.
• Provide regular visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, progress notes, and care plans.
• Supervise and document delivery of care by the Home Health Aide as required.
