Job summary

Presbyterian Healthcare Services is seeking a Registered Nurse to join our Outpatient Hospice department in Albuquerque, NM. You will provide compassionate, patient-centered care, manage cases, and coordinate services with a multidisciplinary team to support end-of-life care in the home or community.

The role requires an Associate Degree in Nursing, an NM nursing license, BLS certification, and at least one year of direct patient care.

Qualifications

• Associate degree in nursing and an active NM Compact state license.
• BLS certification is required at time of hire.
• Minimum one year of direct patient care experience; hospice experience preferred.
• Drivers license and auto insurance are required.

Responsibilities

• Provide direct patient care and case management for hospice patients in outpatient settings.
• Develop, coordinate and schedule care through a multidisciplinary team to patients at home or in the community.
• Ensure timely, accurate documentation and communication with the team and providers.
• Precept newly hired nurses and serve as a resource for staff and students.
• Participate in QAPI and performance improvement activities as required.
