Hospice of New Mexico is seeking a PRN Hospice RN Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care to patients and families in Las Cruces.

As an RN Case Manager, you will coordinate services, guide care plans, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life for our patients.

This is a PRN, on-call role with a focus on compassionate nursing in a supportive environment.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification as required by state.
• Valid driver's license and reliable transportation.
• Spanish speaking preferred but not required; English fluency required.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Coordinate direct patient care and support comfort and quality of life.
• Provide nursing care grounded in proven principles.
• Educate and advocate for patients and families in care planning.
• Participate in on-call duties as needed.
