Qualifications

• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload.
• Obtain data and assess necessary information.
• Initiate communication with the attending physician and team.
• Provide regular visits to re-assess the patient's needs.
• Perform clinical pronouncement of death.
• Maintain accurate patient records and preparation of care plans.
• Supervise delivery of care by Home Health Aide as required.
