Job summary

Hospice of New Mexico is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our nursing team in Albuquerque. You will provide hands-on hospice nursing care, coordinate services, and guide patients and families through the hospice journey.

Qualifications

Hold current unencumbered RN license.
Minimum one year experience as RN in medical-surgical/acute care; hospice exp is a plus.
Computer proficient; EMR experience.
Current CPR certification if required by state.
Valid driver's license, vehicle insurance, and transportation access.

Responsibilities

Empower Through Engagement: respond to clinical referrals with care and expertise.
Collaborative Care Champion: partner with Clinical Director to assess eligibility for hospice services.
Nurturing Direct Care: deliver hands-on hospice nursing care and ensure comfort and quality of life.
Nursing with Heart: apply nursing skills to ease pain and provide compassionate care.
Educator and Advocate: educate patients and families to participate in personalized care plans.
Call of Compassion: participate in call duties to be there when it matters most.
