JourneyCare Hospice in Carol Stream, IL is seeking a compassionate Hospice Registered Nurse (RN) Case Manager for a full-time role. You will oversee a dedicated hospice caseload, perform patient assessments, admissions, and care planning to ensure comfort and dignity at end-of-life.

The position offers a Monday–Friday 8:30 AM–5:00 PM schedule with one weekend rotation every 12 weeks in the Carol Stream territory, plus competitive compensation and benefits.

Qualifications

Active Illinois RN license and CPR/First Aid certification.
Experience in hospice or home health care preferred.
Strong clinical judgment and clear communication skills.

Responsibilities

Manage a dedicated hospice caseload as the primary RN Case Manager.
Perform patient assessments, admissions, and care planning.
Provide skilled nursing focused on comfort and dignity.
Coordinate care with physicians and interdisciplinary team.
Conduct routine visits and adjust plans as needed.
Educate families and provide end-of-life support, with proper documentation.
